Request a Death Certificate
To obtain a Death Certificate for an individual who died in Tolland, MA:
1. Name of the Deceased: List the name and date of death (if you know it) of the person.
2. Make check payable to Town of Tolland
Send $5 check payable to the Town for each certified copy you are requesting
3. Mail Request and Check to:

Make your request at Town Hall:
You may visit Town Hall during normal business hours of the Town Clerk and request death certificates in person.
